Application for Pre-Need Determination of Eligibility for Burial in a VA National Cemetery – VA Form 40-10007
This is NOT a pre-registration or guarantee. It is simply a form to confirm you are eligible to be buried in a National Cemetery.
Download the
va40-10007 and complete the form. Instructions are on the second page of the form including what supporting documents are required when submitting the application. (Updated form)

This application is for Veterans/Service Members, spouses, and unmarried adult child.
There is a requirement that each person (Veteran and Spouse etc.) must submit a form. (I, Webguy, submitted the 2 copies I needed for myself and my wife.)
